KINDERGARTEN REGISTRATION

To be eligible for Kindergarten in September 2025, a child must be five years of age on or before December 31, 2025

For Newcomer Families - Authorization for Release of Information

Please follow these instructions for filling out the online registration forms.
1. Click on the link at the top of this page to access forms for your school
2. Download and save the forms to your computer/device
    ( you must download and save the forms before starting to fill them in or your information won't save on the form)
3. Complete the applicable forms and re-save them to your computer/device
   (we will get your signatures at a later date)
4. Create an email to the school (see email addresses provided below) and attach your saved completed forms
    If possible, attach pictures of your child's birth certificate and for newcomer families, pictures of your child's passport and residency
     documentation to your email also. If this is not possible we will get copies at a later date.

If you would like to fill out, print and drop off the forms please:

print the forms and fill them in by hand or
type in the information then print forms

Completed forms can be dropped off at the Beautiful Plains School Division Office at 213 Mountain Avenue in Neepawa or
mailed/emailed/dropped off directly to the school. See addresses below.
